Teleconsult helps authorized physicians and clinic staff fill in a Medical Certificate and Rx form from a pasted SMS, save reusable templates, and create, print or share PDFs. It is a convenience tool for preparing documents. It does not provide medical advice, diagnosis or treatment.
The physician remains fully responsible for the content of every certificate and prescription, including the diagnosis, remarks and medicines. Always review all fields before printing, signing or sharing. The app may make mistakes when reading an SMS, for example with names, dates or ages.
The app is for licensed physicians and authorized staff of Life & Health Laboratory or its affiliated clinics. You are responsible for keeping your device and Google account secure.
You agree to handle patient information lawfully, including under the Data Privacy Act of 2012, and to share documents only with people entitled to receive them. Please read our Privacy Policy.
Templates are stored in your browser and, if you choose, in your own Google Drive. You are responsible for keeping backups. We are not liable for lost templates, for example if browser data is cleared.
The app is provided "as is" and "as available" without warranties of any kind. We do not guarantee that it will be uninterrupted or error free.
To the fullest extent allowed by law, Life & Health Laboratory is not liable for any loss or damage arising from use of the app, including errors in documents that were not reviewed before use.
